Home Health Care by Caring Hometown Professionals

In-Home Care

Home healthcare includes a variety of services allowing patients to receive top-quality medical care in the home setting.  Services are available for all ages, from infants to senior citizens.  Home care may shorten the length of a hospital stay, eliminate the need for a nursing home stay and assist in the management of a medical condition or medication.

Claiborne Home Health Care provides the following services:

  • Skilled Nursing
  • Occupational Therapy
  • Speech Therapy
  • Home Health Aides
  • Physical Therapy
  • Hospice
  • Medical Social Services

Hospice Care

Hospice provides supportive care to terminally ill patients who have a life expectancy of six months or less.  Hospice provides palliative care, meaning efforts will be made to keep patients as comfortable as possible with primary emphasis on preventing, reducing, and controlling pain.

The following services are covered under our Hospice plan:

  • Skilled Nursing
  • Physical Therapy
  • Occupational Therapy
  • Medical Supplies
  • Chaplain
  • Bereavement Counseling
  • Medical Social Services
  • Home Health Aide
  • Respite Care
  • Palliative Medication
  • Inpatient Care

While family members or other care givers attend to the patient on a daily basis, members of the Hospice team make regular home visits to offer medical, emotional, and spiritual support.  In addition, a nurse is on-call 24-hours-a-day, seven days a week.
